COVID-19 LIABILITY RELEASE
By entering onto any property of Antioch Missionary Baptist Church or participating in any of Antioch Missionary Baptist Church’s activities/programs, I (on behalf of myself and my family members) acknowledge and agree with the following:
The novel coronavirus, COVID-19, is a worldwide pandemic and has proven to be extremely contagious. The virus is believed to spread through the air from person-to-person contact and/or by contact with contaminated surfaces and objects. It has been shown that people can be asymptomatic (infected and show no symptoms) and therefore spread the virus. Evidence has shown that COVID-19 can cause serious and potentially life-threatening illness; and more than 550,000 people have died in the United States. Effective vaccines are now available, and we pray you have been vaccinated. The best defense is to get vaccinated. Wear a mask and social distance for added protection, especially if you have NOT been vaccinated!
Antioch Missionary Baptist Church cannot prevent me or my family members from becoming exposed to, contracting, or spreading COVID-19 while on Antioch Missionary Baptist Church’s property or when participating in Antioch Missionary Baptist Church’s programs. Therefore, by entering an Antioch Missionary Baptist Church facility or participating in one of its programs,
-
I acknowledge that I may be exposing myself and my family members to and/or increasing my/our risk of contracting or spreading COVID-19. I expressly assume that risk, on behalf of myself and my family.
-
On behalf of myself and my family members, I hereby forever release and waive my right to bring suit against Antioch Missionary Baptist Church or any of its affiliates, officers, directors, employees or other representatives related to any of the foregoing.
